Physician-Led Care for Persistent Pain

About Us

Community Chronic Pain

A chronic pain clinic in the community of North York, Toronto.

Assessment by a physician, an individual plan, and a full written report back to the doctor who referred you.

Our Pain Clinic is run by physicians who work as a group rather than in parallel. Cases are discussed between us, approaches are compared, and a patient whose presentation sits closer to one physician’s area of focus can be seen by that physician.

Working as a group also means the clinic holds one standard for how patients are assessed, how a procedure is decided on and documented, and what is written back to the referring physician.

We concentrate on the conditions that most affect our patient population: low back and neck pain, nerve pain including sciatica, myofascial pain, osteoarthritis-related joint pain and chronic headaches.

We approach each of them the same way: work out what is driving it, explain it plainly, treat it according to the evidence, and stay with the patient afterwards.

We Focus on Chronic Pain

Our Team Brings Together

Physician Assessment

Interventional Procedures

A detailed assessment of the pain history, previous investigations, current treatments and how pain is affecting daily life done by a doctor, not delegated.

Targeted injections of local anaesthetic near selected nerves and injections into localised tender areas of muscle, where the assessment supports them.

Medication Management

A structured review of current pain medications: what is helping, what is not, side effects, interactions, and safer alternatives where they exist.

Movement & Education

Guidance and advice on activity, pacing and graded return to movement, with onward referral to rehabilitation as needed.

Care Coordination

A written plan to the referring physician and the family doctor after the first visit, with on-going communication and updates as needed.

Education & Self Management

Time spent explaining what is driving the pain and what can be done about it, so the patient can make decisions rather than just receive them.
